My name is Dave Lewis and I’m a licensed Doctor of Occupational Therapy practicing and teaching in New York State. I have years of occupational therapy experience working in a variety of settings including acute hospital care, subacute rehabilitation, skilled nursing facilities, and long term care. I’m also an adjunct professor in the SUNY Orange Occupational Therapy Assistant program teaching Geriatrics, Medical Conditions, Physical Disabilities and Physical Skills courses.
As an occupational therapist I have experience and expertise assessing individualized client factors, impacts of health conditions, functional independence, mobility, and home safety. I also have experience in activity and environmental modifications, adaptive equipment and durable medical equipment training, and caregiver education. My goals in creating Hudson Valley Home Accessibility are to provide home safety assessment and evaluation, fall prevention and training, and home modifications.
Certified Fall Prevention Specialist- As a CFPS Occupational Therapist I am an expert in the skills and knowledge of interventions to reduce fall risks, minimize fall impacts, and lower the chance of injury for my clients.
Certified Home Accessibility Therapist - The CHAT advanced certification provides expert training in client factors and environmental assessment, safety and accessibility assessment, home construction, design and modifications.
My name is Elizabeth Lewis and I am a registered Nurse with a Bachelor of Science in Nursing from Long Island University Brooklyn. I bring over 12 years of dedicated experience specializing in orthopedics and trauma, sub-acute rehab management, and assistant director of a skilled nursing facility. I currently serve as an inpatient wound care nurse while pursuing a graduate degree as a psychiatric mental health nurse practitioner.
My extensive experience in acute, sub-acute, and long-term care has provided me with unique insights into the critical need for enhanced home accessibility and fall prevention measures. I've seen the impact that a well-adapted home environment can have on the safety and quality of life for individuals. This understanding drives my commitment to creating innovative solutions that promote independence and prevent injuries in the home.
Through my work, I aim to bridge the gap between clinical care and home safety, ensuring that every individual has the support and modifications they need to maintain dignity, and live comfortably, securely and independently.
